The Medical Report Form is a list of questions about your physical and mental health. You need to complete this questionnaire to obtain a medical declaration.
What is it?
The Medical Report Form is a form with questions about your health. Sometimes you need to include a doctor’s certificate as well. You will need the Medical Report Form when applying for a driving licence for the first time or, in some cases, when you want to renew your driving licence.

How does it work?
You need the Medical Report Form to obtain a medical declaration from the Central Office for Motor Vehicle Driver Testing (CBR). A medical declaration is a document stating that you are of sufficient physical and mental health to operate a motor vehicle.
What are the conditions?
There are different types of Medical Report Forms:
- Medical Report Form 70-
Complete this form if you:
- want to do the driving test at the CBR for category A, B or BE
- are issued a driving licence with a shorter validity period on medical grounds
- want to exchange a foreign driving licence in category A, B or BE. - Medical Report Form 70+
Complete this form if you:
- are aged 70 or older
- are aged 65 or older and your driving licence expires after your 70th birthday. This applies to all driving licence categories. - Medical Report Form CDE
Complete this form if you:
- want to extend a driving licence in category C, D or E
- want to do the driving test for category C, D or E
What does it cost?
The Medical Report Form costs € 23.80 in 2013.
How long does it take?
At least 2 weeks. The CBR will contact you.
How do I apply / What do I need to do?
- You may purchase the Medical Report Form from a sub-municipal office, the CBR in Rijswijk or your driving school
- Complete the Medical Report Form.
- You or your driving school should then send the form to the CBR, with a doctor’s certificate if applicable.
- The CBR will review your form and decide whether to issue the medical declaration.
- The CBR will register your medical declaration and send you notification of this.
- You may then apply for a driving licence from your sub-municipal office.
